Getting started with Homegrown Therapy

Every family’s path may look a little different. These are the basic steps that help the team begin the conversation.

    Share basic information

    Contact Homegrown Therapy so the team can learn more about your child and your family’s needs.

    Arrange a referral

    Ask your child’s primary physician to send a referral by fax using the number below.

  3. 3

    Complete paperwork

    Homegrown Therapy will provide the appropriate intake materials and instructions for returning them.

    Provide insurance information

    Follow the secure instructions provided by the Homegrown Therapy team for insurance documentation.

For physicians and providers

Referrals can be sent directly to the Homegrown team.
